What You Receive: Employee and employer contribute pro-rated amount to IPERS fund, which provides retirement income.

Health and wellness benefits
Benefit: Midlands Choice – Plan A
What You Receive: Co-insurance is 80%-20% coverage for all in-network providers. 50%-50% coverage for out-of network providers. Office visit co-pay of $15. Plan also includes dental care, vision care and drug card. $750 single plan deductible and $2,250 family plan deductible. Employee’s monthly contribution is $77 for single coverage, $258 for employee plus children, $272 for employee plus spouse, and $431 for family coverage. Prescription retail $5 or 25% (whichever is greater), max $125. $10 or $70 mail order option.
Benefits: Midlands Choice – Plan B
What You Receive: Same co-insurance coverage as Plan A, but with higher deductible ($1,500 for single coverage, $4,500 for family coverage). Office visit co-pay of $25. Employee’s monthly contribution is $57 for single coverage, $191 for employee plus children, $201 for employee plus spouse, and $318 for family coverage. Prescription retail $5 or 25% (whichever is greater), max $125. $10 or $90 mail order option.
Who Receives: Full-time (32-40 hr/wk) and part-time (16-31 hr/wk) employees

Benefit: Dental / Vision Only
What You Receive: The same dental/vision coverage as with the full medical plan. Full-time employee’s monthly contribution is $18 for single coverage, $38 for employee plus children, $40 for employee plus spouse, and $49 for family coverage.

What You Receive: If you enroll and obtain program requirements, you receive $25 reduction in your health-care premium per month or paid $25 per month (less taxes) annually. Those employees who utilize tobacco products will not qualify for health-care premuim credit but may be reimbursed up to $300 annually for smoking cessation aids.

What You Receive: Allows employees who elect health-care coverage to make contributions exempt from FICA, federal and state taxes. Employees may also establish flexible spending accounts which allows them to make tax-exempt contributions for unreimbursed medical and/or dependent care expenses. Debit card has $6 annual fee.

Benefit: Short-Term Disability
In the event of a short-term disability or illness, employees are eligible for 60% of their basic weekly earnings,
for a period not to exceed 11 weeks.
Who Receives: Full-time and part-time employees (a minimum of 16 hours/week)
When Eligible: On the first day of the next month following a six-month probation period, following a 14-day elimination period.

In the event of a disability, employees are eligible for 60% of their basic monthly earnings to age 65.

Benefit Holiday Premium
Employees who work on designated holidays (New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day) will receive 1.5 times their base rate for hours worked. Employees may also request PTO time to be paid.
